The Housing and Services for Persons with Disabilities Online ClearingHouse is paid for with 100% federal funding from a Real Choice Systems Change Grant awarded to the state of Texas by the Centers for Medicare and Medicaid Services.

Affordable Housing Resources Housing and support services for low-income households and persons who are homeless. Services include homelessness prevention services, rental assistance, utility assistance, home repair, home accessibility modifications, home weatherization upgrades to control energy costs, and permanent rental housing. Use the Texas Department of Housing and Community Affairs' Vacancy Clearinghouse to find reduced rent apartments.

Healthcare Services
Prevention, diagnosis, treatment, and management of illness and the preservation of mental and physical well-being through services offered by the medical and allied health professionals.

Home Health Services
Assistance provided to individuals in their home or residence, including medical supplies, nursing services, physical therapy, occupational therapy, speech therapy, and personal assistance (attendant care). Home Health Services are directed by a physician and provided by a home health agency.

Mental Health Services
Prevention, treatment and support for individuals and families affected by mental illness. Services may include, but are not limited to education, therapy, counseling, rehabilitation, peer support and inpatient hospitalization. Mental health services are provided by community mental health centers.

Substance Abuse Services
Treatment and prevention programs for individuals and family members affected by substance abuse or addiction. Services include community-based, inpatient or residential treatment, education, peer support and other assistance to help people overcome addiction and begin the process of recovery.

Independent Living Support Services
Home and community-based programs and services to promote self-sufficiency for people with disabilities through improved mobility, communication, personal adjustment and choice.

Protection & Advocacy Services
Programs and organizations that provide services to and advocate for the safety and well-being of individuals with disabilities and prevent abuse or neglect.
